Vacancy Nursing for Hope South Africa General project information Project name: New Hope Care Hospice, Grabouw Project type (practical/research/policy): practical Intended for interns from following fields: General Nursing or Palliative Care Nursing background Total project duration: Ongoing Preferred starting period: flexible Number of students required: 2 Project description Background of the project: New Hope Care Grabouw is a New Hope project & Hospice facility situated in the community of Grabouw in the Western Cape, 60km’s outside of Cape Town and serves a peri-urban & fruit farming community with a population of about 65,000 people. Grabouw has a known HIV infection rate of 13.8% and a high TB infection rate of 10% (2012 Stats). 40% of households survive on less than R2000 ($140) a month. New Hope Care Grabouw offers the following nursing & health services: Provision of a 24hr Holistic Palliative & General Nursing Care for patients with HIV/AIDS, TB, Cancer & terminally ill in its 7 bed In Patient Unit. Impeccable follow up care & Adherence support & follow up care through daily home visits in patient’s homes to ensure that vulnerable patients are cared for & maintain their health, morale and dignity. This service is offered by our Home Based Care Unit Patient assessments & referrals of patients to the appropriate Hospital facilities Assisting of patients & their families in the community to have access to health care management by providing transport to patients who are too ill to get to the Grabouw Day Hospital or ensuring that they access the clinical care management that they need. Provision of Health Education and information for patients to assist them with improving their quality of life and ensure that they have the knowledge to be able to make wise and informed decisions Assistance for patients in accessing nutritional or social grants where needed Provision of Spiritual & Emotional support & care for the patient and their family as they deal with the impact of their illness/disease Raising HIV/AIDS & TB awareness in the community through health education talks, testing & screening services which are offered by our Health Counselling & Testing Walk in & Mobile Unit. The testing services offered are free and are aimed at helping the community to turn the “tide against HIV/AIDS & TB” in the community. Problem Statement: Very high infection and disease rates in the community Goal: To help turn the “tide against HIV/AIDS & TB” in the community of Grabouw through the provision of our Palliative Care services. Provide “Hope through Caring”!! Role of the Intern: To work as directed under the supervision of the Nursing Managers in the In Patient & Home Based Care Units.
